How to complete a request with 2 profiles for both referrer and family contact ?

Often the first contact is made by a professional caregiver (social service hospital, primary care, etc.). After that, further follow-up is made with a family or informal caregiver directly.

Move to referrer

In case your first contact is a professional caregiver, you can easily move him/her to the role of 'referrer' in order to register a family member as a contact in the request.

You will need to complete any missing fields in the process.

Please note: if a referrer already exists on this request, you will be asked for confirmation to replace it. If you do not agree, the original referrer will be retained on this request.
Click Replace to link the profile as a referrer on this request.

Add additional family contact profile

Next, you can add an additional family member's profile as the future contact: to do so, either select an existing profile from the search function or create a new contact profile.
Complete the profile until all required fields are filled in, choosing the appropriate relationship to the prospect. Click on Save to then save this profile in the application as a contact.
